Travel Health

Do you need vaccinations?

The practice provides a travel service to our permanently registered patients. Our travel appointments are limited so a travel questionnaire must be completed and handed in to reception at least 6 weeks before you travel.

Some vaccinations require more than one injection to provide adequate protection so 2nd & 3rd appointments maybe required.

We cannot provide travel advice without receipt of a completed travel questionnaire. Travel questionnaires can be completed by clicking on the link below or collected from reception.
